EDIT: the title should read: "<init action="..."/> doesn't work", but for some reason this web site left out the init part...
This doesn't work for me:
<init action="headphone_volume 30% & master_volume 70% & deck 1 level 0% & deck 2 level 0% & deck 3 level 0% & deck 4 level 0% & deck 5 level 0%"/>
...but as far as I can tell from the info on this web site, the code is correct and complete. Can anyone see an error?
I placed this code at the end of the xml, just before the </skin> declaration.

try <onit or <oninit cant recall which one works.
